Fax the completed application to the hiBox Customer Service Center
for verification (fax: 02-23118186, ctel: 0800-080-365), and mail relevant documents to us within 3 working days |

Receive and send email through webmail or enable the Outlook email function according to the instructions in the hiBox user manual. |

After you create accounts and notify us, a hiBox engineer will transfer DNS and configure domain name. |

Once you have passed hiBox verification, hiBox will open an administrator interface for you to set up your account, and to send completion notice. |
|

To apply for a Fax Mailbox as a personal user, please contact a Chunghwa Telecom Operation Center for verification. For further information, or to set a verification appointment call 0800-080-365. |

Whether you are a first-time applicant or a current hiBox Fax Mailbox user applying for additional accounts, you must fill out the application, following the instructions listed below. Chunghwa Telecom reserves the right to deny your application if there is incomplete or incorrect information.

Please include one landline number in the contact number column (the number must be a Chunghwa Telecom number registered under the name of the customer or the deputy). |

If you have never applied for any Chunghwa Telecom products before, and you want to apply for any of the products mentioned above, you will need to go to a Chunghwa Telecom Operation Center for multiple ID verification. Take your ID card and the National Health Insurance card with you. |

I. Setup Fees:
|
There is a NTD500 setup fee for using a current domain name as part of your email address. |
|
If you apply for five or more annually paid accounts, or ten or more monthly paid accounts in your first application, the NTD500 setup fee will be waived. |
|
If you are a larger customer (over 100 accounts), we’ll take your application as a special project, and the price will be negotiable. |
|
Number selection service is applicable only to first time Fax Mailbox applicants. If you are an existing customer, please understand that we do not provide service to change numbers. |
|
II. Identification Information:
|
Provide corporate and company owner’s seals (invoice stamp excluded) and copies of the company owner and contact person’s ID cards (front & back). |
|
Applicants with the outdated version of the National ID Card must apply for the newest version of the National ID Card before we will accept the application. |
|
If you are not a Taiwanese citizen, provide a copy of your valid passport or ARC. |
|
A business must provide a copy of its Business Registration Certificate, while a non-profitable organization (foundations, public associations, etc.) is required to provide a Unified Business Number Registration and Legal Foundation Certificate, both stamped with the official seal of competent authorities. |
|
Offshore companies will need to provide related certificates issued by Taiwanese governmental offices (e.g. Unified Business Number). |
|
If you want to use an exclusive domain name, it must be registered under the name of the business. If the domain name is not registered under the name of your company (e.g. under the name of your mother company, etc.), you must complete the following procedures before submitting your application.

Provide a Domain Name Authorization Form (e.g. a mother company authorizes her branch office to use the domain name) and send the hard copy and supporting documents (document proving the identity of the domain name holder) to the following address: hiBox Customer Service Center

To transfer ownership of the domain name (eg. the mother company can transfer the domain name to its branch office), contact our Domain Name Registration team, and provide documents proving the identity of the domain name holder. |

To obtain related documents proving the ownership of the domain name, connect to the registration website and print out the basic information of the registrant with domain name shown on it. |
|
|
The record for losing ID it will not affect the application. |
|
III. Amendment Request:
|
If the applicant has been blacklisted, the application will be rejected (e.g. a deputy with bad record, an individual or organization using a name of a dissolved company illegally, an application verification required by notification from law enforcement departments, a case ruled by the court as surreptitiously using another company’s name without being authorized to do so, a member of a fraud group, etc.). |

If you are applying for a Fax Mailbox, the contact telephone number needs to be registered under the name of the company, the owner, the owner’s direct relative or spouse. If you do not have a landline number, you can also provide a Chunghwa Telecom cellular phone number under the same criteria as above. |
|
If there any overdue service fees on your Chunghwa Telecom account, they must be paid before submitting the application. Chunghwa Telecom reserves the right to deny the application. |

If you want to stop using our service due to company dissolution, you must provide a Company Dissolution Registration as proof. |
